Making an Appointment with a Pulmonary Specialist
Our office hours are Monday through Friday from 1:00 p.m. to 5:00 p.m. When calling for an appointment, please be prepared to provide the following information: patient's name, address, phone number, date of birth, social security number, emergency contact, type of insurance, diagnosis or reason for visit, and referring physician, if appropriate.

What to Bring to Your Appointment
- Your insurance card
- Records pertinent to your medical and surgical history
- Recent radiological images (X-rays, CT Scans, PET Scans, MRIs) and reports
- A list of current prescriptions and/or over-the-counter medications you are taking, including dose and frequency
Payment and Insurance Information
UCI Lung Center is a provider for Medicare and most all major insurance plans. We provide insurance billing. Please be aware that any costs not covered by insurance are your responsibility. If your visit requires prior authorization from your HMO, please call UCI Patient Referral 714.456.7001
For services that are not covered by insurance, we request payment at the time of your appointment. Your insurance company may also require you to pay a co-payment at the time of your appointment. When necessary, our staff will work closely with patients who require a payment plan.
